Book Report Form

I mentioned in another post that I created my own book report form for our homeschool. We use it for third and fourth grade. I finally have it available to share:

Book Report Form

The first page is the cover page of the report, with a space for an illustration of a title character or a favorite scene, and the second is directions for writing the report. After it is written, the report can be attached to the cover page. I like to use Donna Young's paper, specifically the "plain notebook paper" for the final draft of the report.
